We can only accept new patients who live in our practice area. To get an overview of our practice area and see the boundary, please see the map below.
You can register with the surgery by completing a registration form (GMS1) which can be downloaded here and by bringing this into the surgery (patients cannot register with the surgery online). You will also need to print, complete and bring in an additional information which are listed below:
Proof Of ID (DOCX, 146KB)
Summary Care Record (DOCX, 16KB)
Adult Registration Form (DOCX, 35KB)
Children's Registration Form (DOCX, 29KB) (If a the children is a newborn baby, please also bring the red book).
Your NHS number can be obtained by contacting your previous GP (we cannot register you until we receive this).
When you register, we will ask you to make an appointment to see the healthcare assistant for a new patient health check. This will ensure that our records are accurate and all routine checks to promote your health and wellbeing (such as blood tests/blood pressure/smear tests) are up-to-date.
Due to the pressure on the surgery currently, we will not be able to accept patients who do not live within the catchment area of the surgery (please enter your details in the post code checker below [top right corner of the map], to see if you live within the catchment area of the surgery).
If you move outside the catchment area you will need to unfortunately find a alternative surgery.
